Well this font could be VCR OSD Mono click here or Camcorder click here it's hard to say because the image is hella blurry, but these fonts looks pretty close.

And for the glitchy effect, it's not to hard to make, only use the motion blur filter in Photoshop and that's all, also you can add some noise and a glitchy texture, play with the curves, remember that if you want to do an old vhs film effect you have to distort the picture as most as you can.
