This looks like it, if you add a bit of skew.

I think Azonix was the typeface that the sample was based on. The designer added the dots for the A and the I, and compressed it horizontally a bit. By the way, the dots which imply the crossbar in the A vary in postioin, implying that the sample probably did not come directly from a font.
