Lubalin Graph  Suggested by pilaster 

Here is a real puzzle for me. This looks like Lubalin Graph, but as far as I can tell there is no digital version available with the alternate Ms so either this is;
a) Modified
b) Very old (and done with letraset. See sample of letraset alternates here (I'm sure i remember the alternates in bold in a catalogue)
